The Index of Orthodontic Treatment Need (IOTN) is the system used to determine eligibility for NHS-funded orthodontic care. It ensures that only cases with significant dental health needs receive treatment under the NHS.

What is the IOTN?

The IOTN is a grading system that helps decide whether a patient requires orthodontic treatment for health reasons. It is split into grades 1 to 5, with grades 4 and 5 being eligible for NHS orthodontic referrals​.

  • Grade 1 & 2: Minor issues, no treatment needed.
  • Grade 3: Moderate issues; some cases may qualify, but only if an aesthetic score is high enough.
  • Grade 4 & 5: Severe orthodontic problems that impact dental function, making the patient eligible for NHS treatment​.

Who Can Be Referred for NHS Orthodontic Treatment on the Isle of Man?

The Isle of Man Department of Health and Social Care (DHSC) follows the UK system, meaning only patients classified as IOTN Grade 4 or 5 can receive NHS-funded orthodontic treatment​. Common conditions that qualify include:

Severe crowding – when teeth overlap significantly.
Large overjet (protruding teeth) – where the upper teeth stick out more than 6mm.
Reverse overjet – when lower teeth bite in front of upper teeth.
Impacted teeth – when adult teeth fail to emerge properly.
Cleft lip/palate or craniofacial anomalies – automatically qualify.

Children under 18 with severe orthodontic needs can be referred through their general dentist, who will assess whether they meet the NHS criteria before making a referral.

What If I Don’t Qualify for NHS Treatment?

If a patient is IOTN Grade 3 or lower, NHS funding will not be available. However, private orthodontic treatment is still an option. Private treatment offers more flexibility, shorter waiting times, and a wider range of braces, including clear aligners (Invisalign) and ceramic braces for a discreet look.
